MEDIA/NEWSLETTER RELEASE FORM

This information will let you know your preferences for publishing your picture in our newsletter and social media sites (church website and Facebook).  It will also help us update our church records.  We hope to be able to start putting birthdays and anniversaries back in our newsletter after we get everyone’s response.   Thanks for your help!
